You can find the terms and acknowledgements of the NASW-Michigan Chapter CE Provider Application below. In order to submit your online application, you will be required to agree to these conditions.
Application Information
- Submission of your application, including application fees, does not guarantee that your application will be approved. Should your application be rejected, you may be given a chance to re-submit your application with corrections and/or additional information as requested by our office.
- Application fees are non-refundable. Should your application be rejected and you are unable to re-submit a corrected application, you will receive a future credit in the amount of your application fee (excluding any paid rush fees).
- If we determine that you paid an insufficient amount as a result of improperly completing the form, your application will be placed on hold. Your application will be considered "submitted" once you remit the outstanding balance remaining. This may also result in either an additional mandatory rush fee being applied to your application or your application being rejected - see #4 below for more details.
- Applications must be submitted at least 30 days before the start of the proposed CE event. Applications submitted 61+ days from the date of the proposed CE event are not subject to rush fees. Applications submitted between 46-60 days from the date of the proposed CE event are subject to a mandatory $75 rush fee. Applications submitted between 30-45 days before the start of the proposed CE event are subject to a mandatory $100 rush fee.
- You must submit a new CE Provider Application for each new CE event you are seeking accreditation for. This includes Unlimited Providers that have previously paid their annual dues.
- Regular applications are reviewed on a first-come, first-serve basis.
